Dual-head graphics  
Do we have graphics drivers that support dual-head graphics cards, e.g. like matroxg?

The QNX4 graphics framework doesn't support multi-head cards.  The only way to get multi-display is to have multiple 
graphics cards.   The only drivers that are known to have worked in a multi-card setup with QNX4 Photon are Pg.rage128 
and Pg.radeon.

in a multi-card setup? (i.e., what is the reason?)

At power-on only the primary card gets initialized by the BIOS.

For a driver to work as a secondary device this extra initialization needs to be done in the driver.   Only Pg.rage128.
so and Pg.radeon have this capability.

There are a couple of reasons for this including specific requests for therse devices, availability of requried 
documentation and HW,  and generally multi-card has been a low-runner use case.
